what happened here? [resolved]

Not exactly posting a problem, but I recently found a html form created by me when I was eight years old. I can't exactly figure out why it's doubling certain elements, or how I got overlapping textboxes?

Anyone understand?

You have the form in the code twice and both are inside of divs using absolute positioning.
