Attention PHP Experts! Need Custom Type Script

I am in need of a script, something like a guestbook or a feedback where members can post restaurant reviews. I know absolutely nothing about PHP or I would tackle this myself.

What I would like is a script that, for the input page, has nothing more than the Member Name, the restaurant name and their review. I would also like to have the capability, as admin, to delete posts and some way of discouraging spam.

I would like to have the output display on a seperate page. I would like to be able to set how many posts show per page. I would like to have the output show the Member Name, Restaurant Name, their review and have the post date/time stamped.

Does anyone know of a free script that will do this? If there is not a free script out there somewhere, then how much would it cost me to have someone write this simple little script for me? Keep in mind that I have a VERY SMALL budget (like pennies on the dollar) for this if I have to go this route.

I need this for a website and it's launch has been on hold for the past few weeks because I've been trying to figure this problem out.
