Could it be an issue with a specific theme you are using? Some themes do not always play well outside of their own sandbox. Perhaps try a different theme and see if you encounter the same issues?
Could be another plugin that's not getting along with this one or something else. If there is an issue, the plugin author would be able to make changes to the plugin right away.
I have had a Similar problem. I found if all the images aren't in the right folder or the if something is pointing to the wrong place it won't work.
Try looking in the js file and make sure the folders are pointing to the right place.
