Thumbs down How to add secure approval forms to a site?

I'm new to this forum and look forward to learning and sharing here. I wasn't quite sure where to post this question, but this section seemed like a good guess. Sorry if it should be somewhere else.

I'm in need of some help with a current client's website. I need to add a form that can be 'approved' or 'declined' like many of the TOS we have to accept to use or register for some websites (like this one). How do I add secure forms like this?

I don't know what I need to know to set this up on this website.

The client is a property management firm and needs property owners to be able to log in and approve forms for the management company to do repairs on the owners properties. This is to replace forms that have to be faxed back and forth for signatures.

I am not a coder or programmer at all and I can get a little lost in the technical stuff, so if you can direct me to where I can learn this simply, or to someone who could do this for me (!), I would really appreciate it!

I need to get this done as soon as I possibly can. Any help is very much appreciated.

Is there really no one who knows anything about this, or could point me in the direction of someone else who would know?
