Making a PDF File Fill-Inable

Hey all
I have this PDF form for a class and on the side it has some lines so that if you print it out, then you can write on the lines. However, I was wondering if anyone here knew how to make it so that the form was typable on like this document:

http://www.irs.gov/pub/irs-pdf/fw9.pdf

Anyone know this?

No, i didnt know open office could work with adobe files?

Open Office can export to pdf but it won't help with fillable fields.
