Contact form issues

could someone take a look at my contact form for me? I know what the problem is but I don't know what to do to fix it. if you look at the contact us form here http://www.essential-oil-aromatherap...actus-form.htm you will see in the code the following

<form method="POST" action="http://www.essential-oil-aromatherapy.net/mailto" onSubmit="return(sub_form(this));">

i'm pretty sure that this is wrong. you will also see

<script language="JavaScript" type="text/javascript"></script>

i think this may be wrong because before there was a URL to a university with scr=http://www.bla-bla-bla.com. i took this out.

any help?

thanks!

They probably have a script in that URL, that sends you the email when an user submits the form, so you should leave that as it was, and change only the following lines:
HTML Code:
<!--2. ASSIGN A VALID E-MAIL ADDRESS FOR MAILTO VALUE. Change the mailto value="" to a valid UD e-mail address or UD departmental address THIS IS A REQUIRED FIELD--> <input type="hidden" name="mailto" value="ron@orbitelcom.com"> <input type="hidden" name="mailformat" value="text"> <!--3. CONFIGURE THE HTML_SOURCE (RESULTS PAGE). Change the html_source value="" to the URL where your results page is located. Be sure to include a fully qualified path, starting with http:// THIS IS A REQUIRED FIELD --> <input type="hidden" name="html_source" value="http://www.essential-oil-aromatherapy.net/contactus-form-results.htm"> <!--4. CONFIGURE RETURNPAGE (THANK YOU PAGE) Change the returnpage value="" to the URL where your thankyou page is located. Be sure to include a fully qualified path, starting with http:// THIS IS A REQUIRED FIELD --> <input type="hidden" name="returnpage" value="http://www.essential-oil-aromatherapy.net/contactus-thankyou.htm"> <!--5. CONFIGURE CC VALUE. Change the cc1 value="" to a valid UD e-mail address or UD departmental address. You may also add up to 10 cc's by numbering them in sequence. i.e. cc2, cc3, etc. THIS IS AN OPTIONAL FIELD--> <input type=hidden name="cc1" value="renee@orendainternational.com"> <!--6. CONFIGURE SUBJECT DISPLAYED IN E-MAIL. Change the subject value="" to something related to your contact form. This will appear as the subject in the e-mail that you receive. THIS IS A REQUIRED FIELD--> <input type="hidden" name="subject" value="A Potential Distributor" maxlength="1024"> <input type="hidden" name="suppress" value="mailto returnpage serverinfo answer">
Cris.
