Question Best Way to Code a PSD to CSS?

What are the most effective ways you code a PSD file to CSS (tableless)?

I see Image Ready can do it, but not with the best code. Do you do it by hand or pass it through a program and tidy it up? I've seen some services online where they will do it for you in a day, so they are either really fast coders by hand or they have some program they use.

I think they are "really fast coders by hand"

Mock it up in photoshop then slice it up. Create all your divs to accomidate the slices, sized according to slices. Use the smallest images possible and fill in the blanks with hexidecimal colors. Use floats and margun to bring things in place. There's no rules in CSS, just directions to make the page the way you want it to look. Talk it out to youself and give the page directions.

Take a look at sitegrinder

If you do a search for sitegrinder on google, this software bolts into photoshop and converts it to a css design. I use it alot as it speeds up the process. Before I used fireworks but it slows things down considerably.
