States Drop-Down Box

I am creating some forms for a Travel Agency. The user needs to choose their departure and destination city and state.

I need to put in drop down boxes with all the states listed. Does anyone have this?

Seriously, I would appreciate an objective evaluation of my list. For example, should I add the 2-letter codes to the option text?

Looks good... I wouldn't use the 2-letter codes in the option text. I took them out of the one I got from John.

I had one on my site and it highlighted a country or state as the top on that is was on automaticaly. I am loooking for a java dynamic one if anyone can help, one that will start with country then a state box will appear then a city all depending on the one above it.
