adding personal autoresponder forms to affiliate pages

Hi folks,

Does anyone know of a way to easily add an affiliate's personal
autoresponder to a page so that leads go directly to an affiliate?

Right now I am manually creating affiliate pages for members and
affiliates work off of referral id's. Yes, I know there's easier ways
but again, i want to have leads generated by the affiliates go
directly to them.
