Can a user specify the email address to submit a form to?

We're looking for a solution on our intranet to let users submit a form to different email addresses.
For example, if I fill out the mileage request form, I need it sent to my manager. John, who works in a different department, fills it out and needs it sent to his manager.

I have used a dreamweaver plug in for ASP form to mail, but you have to specify the address when you create the page.

Does anyone know of a way to let the user specify the address?

Could we incorporate a dropdown list, somehow, where users would select their managers? Ideally they would be able to just type in their email.

Or a way to do the same thing with a pdf form.
We also can't let users save the completed form and email it, because not all users have internal email.

Any help is appreciated! Thanks.

Check out themikes.com php script. It allows for as many dropdown or hotlink emails as you would like to use.

hmmmm... if you programmed this yourself (which is very simple, it's definitly not more than 20 lines of code), you could set it to send to anyone you wanted.

I looked at themikes.com and contacted them and got this reply...

"How weird. I have no php on my site, just a little js.


Just seems really odd that someone on your forum would be listing my website. Obviously a mistake."

Does anyone know how to make the dropdown pull from outlook's global address list?
