Download simple php contact us form and script
1. Download the attchament zip file. Unzip it.
2. Open contact.php in your texteditor. Change 'YOURSITE.COM' to your website name. (It is in 3 places. Line 37, 51 and 52). Save it.
3. Create a folder on your site called 'contact'. Upload all three files to it.
Access yoursite.com/contact/index.html
Thats it!
It very simple form. (I am using it for someother purpose on my site. I edited a bit to make it simple)
Use: Your visitor contacts you thru this form.
