I cannot speak for everyone, the history is alot of fake doctors would aquire documents from a diploma mill in the Philippines. Passing themselves off as doctors, but were not.

MANILA, Philippines – Manila Mayor Alfredo Lim ordered yesterday a crackdown on shops along Recto Avenue found manufacturing fake documents.
