Over the last few years I've had 2-3 hacks, mainly from not keeping my OS up to date. The worst OS i've used for hacks is redhat, I've never had anyone hack into my debian boxes.

Most hacks these days are fairly harmless, and you'd be hard pushed to find out they've hacked your box, except your box is now connected to an IRC network (netstat helps for this) and are a member of a botnet.

I've had a few defacements as well, but it doesn't happen often.
