I still use sitemaps, yes both of them, even if my sites has good link structure. And by using the Google webmaster tools, I can easily check which of my pages has a broken link.
Mostly, the reason why pages aren't indexed is because of bad link structure. Check again your inner pages, see if each categories link to one other.

Duplicate meta tag? Are you kidding me? I have handled several websites with thousands of inner pages having the same meta tags, yet SEs index them (not all, but most of them). It's all about the link structure and some link building techniques, like social bookmarking that helped.
