I'm right there with ya and even though I do pretty well much of the time in SERP Placement, I'd still love to be more consistent, who wouldn't? I'm sure you're going to find a ton of differing and overlapping opinions on what makes for good on page SEO, I'll just try and elaborate on what I've chosen and why.
I try and observe the following in order of importance:
- Create Site Theme/Site Focus "Relevant" Content: You can be the best writer/SEO Expert in the world and still rank poorly for a well written article that has nothing to do with your sites direction. For example, I run a Family Friendly Website focusing on dealing with Social Issues so if I were to write an excellent and well structured article on After market Car Parts, Google would laugh at me and place me way at the back on result 2,910,001 of 2,910,000.
- Keyword/Keyword Phrase Research: There's really no point in ranking #1, Page 1 for a keyword/keyword phrase nobody is looking for. There are plenty of good, free tools out there that give you a good idea of Search Popularity such as Google Trends and Google Adwords Keyword Suggestion Tool.
- Write what interests you: If you're simply writing to fill the page then you may be doing more harm than good. If you're not interested in what your writing, it will come across in your writings and likely turn people away. You can't engage a reader if you can't engage yourself.
- Make your content unique, informative and engaging.
- Write in order of importance: Write for yourself FIRST, you're audience SECOND and the Search Engines LAST.
- If you're going to use headings, support them: This means that you should repeat your headings somewhere in the article body. Whenever possible I try to do this in the early stages (first paragraph) and end of article (last paragraph) leaving the body to state and support my findings.
- Monitor your articles: Don't publish and forget, monitor your articles to see how they perform. If a particular article doesn't perform as you'd hoped, massage it when it stagnates in the SERPS and monitor the changes. This is THE BEST WAY TO LEARN what works for you.

You'll notice I've stayed away from specific use of Heading Tags, Keyword Density, Meta, etc., there is a good reason for this. Everyone and their dog has a "Proven Formula" for this and I've yet to see anyone back up their claims. I've personally published content that was rich in keyword density (for example) yet placed behind someone who had the same keyword mentioned once in a navbar. Conversely, I have articles that bring me tons of targeted traffic with little in the way of keyword density because of the uniqueness of the topic.
I realize this may or may not be exactly what you're looking for so my apologies if I've missed the mark. The reason I've chosen as above is you can spend tons of wasted time learning the ins and outs of On Page SEO and still rank poorly because part of the main recipe was wayyyy off. All I've attempted to do here is steer you in the right direction from the ground up because starting off in the clouds usually means you only go as far as the breeze will carry you yet you can accomplish nearly anything on a solid foundation.

Every paragraph needs to have a heading or subheading, because visitors scan rather reading from start to the end. Headings should be short max 10 words.
Paragraphs needs to have 50 - 70 max 100 words. Every paragraph needs to have a picture, visitors likes to rest their minds.
Every paragraph should focus on main theme and of course subtheme.

content optimization with 2-4% keyword density and using your targeted keywords is much better also to get rank in SERP. but dont do the keyword stuffing. and best tips is through using of keywords in domain name and use of keywords in your title tags

All things begin with keywords. So, keywords first!
Then, keywords must appear in your Title tag, meta desciprtion tag and page contents (including h1, h2 tags).
