Photoshop Gradient Problems
All of a sudden, whenever I try to use a gradient in a mask, it does not completely remove the "black" area in the mask. Even when I use a brush or fill the mask black, there is still a faint image left. Has anyone had this problem? I can not figure it out.

.. are your opacity and flow rate percentages at 100%?

Hmm .. I don't know. I know that when selecting then deleting dark on a white canvas, I sometimes have to delete 2 and even 3 times sometimes, and I never have figured that out ... I wonder if they're somehow related.

Are there any transparency boxes checked or transparencies locked?
Are whites and blacks pure?

No transparency boxes checked or transparencies locked.
All whites and blacks are pure.

I don't know ..
.. is this anything? Are you using Quick Mask? And are you using a selection? (Or maybe they're talking about a selected area of the layer, selected in the layers palette .. not sure. I'm not very familiar with masking.)
To change the Quick Mask options:
Double-click the Quick Mask mode button in the toolbox.
Choose from the following display options:
Masked Areas to have masked areas appear black (opaque) and to have selected areas appear white (transparent). Painting with black increases the masked area; painting with white increases the selected area.
With this option, the Quick Mask button in the toolbox appears as a white circle on a gray background .
Selected Areas to have masked areas appear white (transparent) and to have selected areas appear black (opaque). Painting with white increases the masked area; painting with black increases the selected area.
With this option, the Quick Mask button in the toolbox appears as a gray circle on a white background.
I think that when you check that "preserve transparency" box, it preserves the transparent pixels of the layer beneath the one that you're editing.

All I am trying to do is fade an image. You click on the image, and then click on the layer mask button. Then I use the gradient tool and make a gradient on the image. All of it does not fade (when it normally does). Then, if I try to fill the whole mask with black (to make the image disappear to test), it is still there, but at a lower opacity. I checked all of the options (opacity of the gradient, etc.) but I can't figure it out.

try to hit and hold ctrl+shift+alt as soon as you click open photoshop, should reset photoshop
