Effective Website Form?
 
My website have 4 PR as well as It's coming up in Google index. But problem is leads. I haven't get leads.

Is problem in content, design, form, structure?

Suggest me: form Design or if possible show me sample web form
because no body fill out the form. why?
Please suggest me if anybody are interesting.

I think it is because you don't offer anything for free. Instead of visitors requesting a project offer try giving away a free report (about something that will be valuable to your clients in general).

E.g. "FREE report on how to cut more than 40% off your next consultant bill"

I'm not good at headlines but I hope you get the picture ;)

Lot of distracting images before the real content.
I think this http://www.telegenisys.com/services.html is a more effective design than the used in the homepage

It's also weird to see the the navbar in the top of the page, before the logo. At least for me.
