I can usually get in the top 3 for anything under 3 million.
Still working on the 18+
I have had many at #1, and even some where every page out of the top 40 were mine.
It is hard work

I don't understand the question.
If your asking if I make the content on the pages I build related to the subject, then yes. If you are asking if I watch how I link to the sites, absolutely.
Why wouldn't I do both?

I was just asking what your main strategy for achieving rankings is. For example, with V7Inc.com, I don't do any page-elements SEO. I rely on anchor text of inbound links for the rankings.
On Suzuki-Bikes.com, the rankings aren't competitive and they are gotten via page-elements SEO.

I completely understand. I do both.
If it isn't competitive then I do content.
If it is I concentrate on links into the site.

I have a #1 for a 2 word phrase (one of the words being 'uk') out of 5,140,000. (I am afraid it's currently only #68 out of 39,700,000 for the word that isn't 'uk' - though it did used to be #42)
Same site also has a #4 out of 5,450,000 for another 2 worder.
Another site has #9 out of 8,940,000 for a two worder. It was #1 pre-florida. Gah!
But as most of my clients are in the UK, I obviously do better for UK searches and don't figure too much in Google.com.
Nothing to do with SEO, but one time in Looksmart, thanks to an editor going overboard on deep-linking, I had positions 1 through to 99 for the word football. Yes, 1-99 for football!!!! Of course, the client thought I was some kind of god. I just did the typical Brit self-deprecation stuff, "Oh, just glad to help, and I do have to rest on the seventh day, etc."

My site is currently:
Number 8 or 10 (bounces back and forth) for the term "nirvana" in Google out of 1,780,000 pages.
Number 1 for the term "nirvana music" in Google out of 1,180,000 pages.
Number 1 in Yahoo for the search term "nirvana" as well...
I'm happy with that
Now to get my PageRank up where it should be...
