Sign in to your Google account.
Go to Disavow Tool Section.
Select your website.
Click the button "Disavow Links"
Choose the file you created for removing.
Then Click "Submit" button

You just have to save the bad links in a notepad doc. Place one link per line. At the top of the list you should include a the sign # and then add the reason why you would disavow the links. And then below it, you post the links.

Sample:

#Poor or spam backlinks
http://www.firstlink.com/specific-url-page/
http://www.secondlink.com/specific-url-page/
http://www.thirdlink.com/specific-url-page/

#Non existent pages
http://www.firstlink.com/specific-url-page/
http://www.secondlink.com/specific-url-page/
http://www.thirdlink.com/specific-url-page/

You can write as many as "# reason to disavow" and post as many bad backlinks as you can.

And then you go to the Google webmaster tool and head to the disavow page.

You simply have to upload the notepad doc.

And that's it!

Ok, If you want to disavow whole domain than see below given samle :

domain: Domain Name1 (without http, do not leave any space between : and domain name.)
domain: Domain Name2 etc.

if you just want to disavow few links than :

link1
link2

make a simple notepad doc. file like this and upload it...

Why would you think that disavow is the way to go for "broken links in my website"?

Fixing broken links and disavowing incoming links are 2 different things. If you disavow urls on your own website that is not a good idea.

Yes,
You can select all the broken links by using broken link checker and download. Add the all urls in notepad and write some program like: https://support.google.com/webmaster.../2648487?hl=en

First you use ahrefs to filter all the links you think are bad.
Next you give all your domains and links to the .txt file.
Finally go to google webmaster tool (disavow tool) to send the file to google and wait for feedback.
