You can just create a text file and name it sitemap.txt and put it on your web server. List all of the pages on your site:
http://yourdomain.com/index.html
http://yourdomain.com/contact.html
............................../whateverelse.html
This is what I have always done. Anyone know of any tools?

Depending on your goal, sitemap MUST be of pre-defined structure. For example, if you are going to create a Google XML sitemap, you should follow this structure.
A sitemap ISN'T just some "List of all pages"....

I use xml-sitemaps.com for generating both yahoo & Google sitemaps. Sitemaps help in easier crawling & indexing of the pages by the Search engines.

You can link. However, if you register yourself into Google Sitemaps, you'll be able to submit your sitemaps there. In this case you won't need any link.
If you want to link from your pages, it must be in XML or (X)HTML format.

Yahoo allows you to submit XML feed. Anyway, if you want to be 100% sure, just put a small link saying "XML sitemap" to the bottom of your pages. Format of file should be supported by all search engines ( http://www.sitemaps.org/protocol.html).
BTW, I wouldn't focus on sitemaps only. It's better to create a site which allows a visitor to open any page by max. 2 clicks from the homepage. Then it'll be SEO friendly and search engines will like it. In this case you don't even need any sitemap. Just avoid 'bad' characted in URLs (spaces, ASCII 127+ chars, etc.) and it will be fine.

Use these link, it will generate html site map, rss and xml, and it's free:
http://www.sitemapdoc.com/ ---> just click on show filter & settings to get the html sitemap after you enter your url and press the create map button.
I've used it already I just edit the generated code with notepad I just view source it and save it and put it in my page....
