Search Engine penalize a site if your site have following :

duplicate content
bad neighbour linking
spamming

When google penalize a particular site, often PR goes to 0. Home page will affect first and slowly the effect gets applied to whole site pages.

To avoid penelization Remeber do not link to a site which has got banned / penelized by SEs. If you have a link partener whose site is having PR0, and you have inbound link from them. Then there is no point in having such a backlink.
Also DO NOT link to a site in link farm, it will cause you penelization.

Also some on-page optimization can lead to penelization. like the tactics heavy keyword repetations in title and in body text. NO Html Validation.

Also you can check the cache for your site. It will show you the last date when crawler crawled your site.

If your site has been penelized it will not see in the SERPs.
Also you will not find any information in site:domain-name.com

Imortant Note :-
New site dont have PR at starting. They take some days even months to get PR.

Nice informative post. Spamming doesn't help in rankings at all. Follow whitehat SEO and you will get better rankings

Almost correct. In the long term spamming will NOT help your SEO. In the short term it can (See SEO contests).

Duplicate content will not cause a site to be penalized, but it *might* cause the content to be filtered out by Google.

Not having valid HTML will not get you banned. Google doesn't care if your HTML is validated or not. As long as Google can crawl your web site with dynamic links don't worry about validation.

Om the other hand, I must say that a website validation using the latest standards (validator.w3.org) will ensure you compatibility with major code readers and content display environments. In this way your website will load faster, will generate less load on server, will be search engine friendly (when looking at various tags and crucial areas of the code that may be missing) and so it will have an overall better performance.

i am not saying duplicate content is the only issue for which your site get banned.
Along with duplicate content there are many more reasons for site to get banned.

It is enough to get in supplemental results and all your beloved traffic will just go away.

I agree. I do HTML revalidation time by time because of regularely update of my site content. Sometimes I don't have enough time to do validation, but I noticed there is no difference whether I do it or not in terms of Google ranking. So I start thinking, may be I shouldn't waist my time on that validation.

I had a site banned back in March, because somehow I left some weird scripting on the sitemap page, was a mistake. I removed the script, not sure how it got on there.

I contacted Google, by End of May my site was back in google better than every. #1

Never link to a spammy site. Check the sites you are linking for quality(content), hidden text, too much links, duplicate content etc.

There is several different peniltys you can get
a slight reduction in page rank your site will drop down a few spots this is just a warning from google
next come more reduction in page rank your site will drop more.
then there is the keyword or phrase penilty you will not list for that pharse or will be kicked to the last pages of the surps but will still have good listing for other phrases
the worst is when your domain get baned and will not even list for its domain name
