Increasing Affiliate Conversion Rate

My hosting review blog, webdedicatedhosting.com, gets good traffic and is high in rankings on google for many good seo terms, but the conversion rate is low. I am planning to have the site redesigned, either with a premium them or with a custom design, but before I do that I was hoping webmasters, experts, and opinionated marketers can provide suggestions that would help. Please tell me what you think would work.

I have a website that I demoted down from a premium theme to a really ugly theme and it worked for me, the CTR just went through the roof.

IMO premium themes are good to start off with when linkbuilding and dong guest posting on other blogs, because most people won't link to an ugly made for affiliate website. But ugly sites with eye catching banners actually encourage CTR. So you may want to try that.

That's the thing, my site isn't premium, but in the "ugly" category. The site is only getting one conversion a month with 1000 visitors. That just seems off compared to my other affiliate sites. This is my only hosting related affiliated program, though. Any advice?

Hosting is really competitive and people in this niche are savvy enough to grasp they just hit an affiliate site. I've seen this numerous times (let me guess, one of those WP review plugins?) and knew instantly this is just a site to pitch me some reviews with your link. No hard feelings, we're all doing that But conversions usually go up in other niches, unless you can really presell properly.
That starts with not using the same theme everyone else is using, but in this niche I wouldn't advise to drop hundreds of dollars on a custom design. It might not be worth it.

Secondly, focus on less hosts. Review 3 different ones and make clear what separates the #1 from the other 2. If you give people too many choices, they will feel overwhelmed and won't choose anything.

Your affiliate links look good though (i.e. not directly like affiliate links) and the site is well structured.
Where is most of your traffic coming from? Is it relevant and targeted?

Drill down in niches. Don't review "hosting". Review "professional lightspeed webhosts", for example. Or "hosting for beginners in internet marketing" (that would be hostgator and co.), know what I mean?
