I didn't submit any adult sites yet, but while submitting so many sites in the past, I've noticed that a lot of sites have an "adult" category. So, I guess it won't be any problem with your friend's site. And, as you said, penis enlargement sites aren't really adult, I would rather put it to "health" category or something like that.
