Hehe. Erm, how do you know? Sometimes you can't easily. Depends on what's been done. Hacked (well, cracked) means to get unauthorized access to a system.

If you find out someone's account password but do little else, that would be "hacked" but no-one would really know.

But obviously if someone started deleting databases/website files, or defacing pages, it'd be pretty obvious that someone was up to no good.

As for your ports question - a server has various 'points' (or ports) which are used by various services inside that server (so FTP, usually on port 21, HTTP, usually on port 80, etc).

*Technically* if a port is left open but not being used, it could be a way of gaining entry to the system. (But with firewalls and all this isn't likely)

Keep an eye on server load, that's usually the most obvious tell tale sign.

On my servers that I have by default I close all ports except for the ones I am using because there are so many ways to exploit unknown programs running on random ports.

don't forget to flush all the Iptable rules after a reboot or server restart most server have a default IPtable, it means that all Port are Open, it is safe to create a shell script that has a default Iptable rule "drop -All" .

when you are hacked you will know your self.
get a good firewall, antivirus and test your code for any injection

if you have nmap installed, it can tell you your open ports
nmap -v localhost(or ip)

a) look at the source coding to see if there's something there that shouldn't be. e.g. hidden links to other sites
b) create a Google Alert to monitor if certain spam words were found on your web pages.
c) use a website monitoring service or software. SiteUp is freeware you can install on your computer. You have to leave your computer on of course to monitor the site while you are away from the computer.
d) Review your indexed pages in the search engines.
