For me its always:
- Logo / Header Banner
- What options are on the menu. Is the navigation any good?
- Are there any "Most Popular" type widgets on the site so I can see it's best content easily
It totally depends on who is browsing the site.
It's hard to get past the graphics and logos etc. though.
If the site has a rubbish colour scheme and a poor logo/header its very rare I explore it any further.

I guess its more or less the same for me, but I also look at how long it takes to load. Sometimes if I have to wait for a long time for images and such to load, I move on to the next site in the search.

Honestly, the first thing I notice about a website is its domain name; after contemplating whether or not I like the domain name, then I will move on to assessing the design.

Page/load speed of the website/blog.
If it takes awhile to load up then i'm gone. Also this was mentioned before, the navigation is what i notice.

Some of my answer are already mentioned, here's the my list: (chronological order)
Loading time
I'm patient, yes, but when it comes in viewing webpage, I become impatient. If the page didn't load within 3-5 seconds, it's faster to close the window / tab where that page reside. 
Logo, banner, themes, layout, and "look and feel"
I wanna see how relevant the appearance of the webpage to the focus of the website, if not I would take some time to investigate why or why not. Also I don't wanna see dark colors as a website's scheme. Including the text style, size and color.
Navigation friendliness
I wanna see how compact, useful and friendly the navigation is. I wanna know if it tells me where I am. Also it must contain some basic element a navigation really must have, not those affiliate links.
Number of present media
One thing that affects the loading of a website is the media placed on the page. Some have auto playing youtube videa / any built-in players (not a good idea, it consume the bandwidth). Esp. the images, some website owner didn't care about resizing the images, as long as they know how to fit it using HTML codes, issue addressed. But no, though the image(s) is (are) shrink to fit the frame, the quality and file size is the same, which also affects the loading of the page.
Number and size of ads
I'm particular in ads placement. I saw some website before, the ads are the same of the thumbnail, which I think is not a good way to gain some clicks. Oh yeah, sometimes I include that website in my block list! How cruel MrSS is!
Content quality
Sometimes I'm so skeptical about the content of a website, so I jump to search engine to know if the content is original, if it's only a copy-paste and not giving any source, then I will close the window right-away and never visit that again.
Links
I wanna see the actual link of what a certain link is pointing, not cloaked in shorten urls like bitly.
Optimized for different monitor size
This could be on the end of user, but it's good to know that a website is viewable in different monitor sizes without scrolling (too much).

If a person is searching for any particular thing on the internet and comes across a website, then firstly he goes for the themes, contents and accessibility options. Navigation key of course is another major thing which a website must have. If a website is large in size then it should be worthy. It should be useful not just a bulk site. Ads and links come later on in it.

Generally, web design is the first thing that users see. In connection, the three areas in web design that netizens immediately notice (or get annoyed if cluttered) are layout, navigation, and color combination.

The first thing thing that I notice about a website is its structure and the color combination used. The logo and the header are also among some of the second line ones but it would totally depend on which among these is the most attractive and impressive in that website.

For me it would be the pageload speed .. and .. how much jumping around it might do as the ads load. ..

Page loading is the first thing anyone notices, but only when it is too slow. As far as design goes, images and video are what draws people's eyes immediately. I don't believe people pay nearly as much attention to logos or to banners anymore...unless they are made with vibrant images. Other then that they are looking for pertinent information that relates to the keyword or advertisement that got them to your site.

Different people has different opinions. I think you site header and navigation bare. These should be clear and appealing.

It obviously depends on the website. If for instance the logo is very small and the navigation bar is right near it, it's bold and huge, well, I'll first notice the nav bar.
Anyway, I think usually we all see the same things first: logo and navigation/menu.
