Help needed with Feedback Form. It just aint workin :(

We've created and uploaded our site which includes a fairly decent looking feedback form (http://www.mjrfm.co.uk/contact.htm

Just one problem though - the feedback form doesnt work - try it - as soon as you click on the SUBMIT button you get this page http://www.mjrfm.co.uk/_derived/nortbots.htm

We've installed the FrontPage extensions from Easyspace (our hosting co.) but still it doesnt work.

We can't contact EasySpace as they only operate via e-mail and they don't respond to e-mails!!

any help would be massively appreciated.

This isn't the answer you're looking for but IMO it's the right answer:

Forget Frontpage extensions. They will always be unreliable. Spend some time learning how to use a CGI script to achieve what you need. It will take some time, but once you've done it you'll never look back. It will save you much much more time than the time you'll waste on Frontpage.
