how to make an online sign up form?

hey guys,

I own a webhosting company. im planning to expand it by letting customers to sign up for the "web hosting plans/service" online. for this, i want to develop an online sign up form with these fields:

Textfield
Name:
Address:
City:
Country:
Email:
Phone#:

Dropdownlist
1) Basic Plan
2) Advanced Plan
3) Business Plan

Textfield
Domain Name - www. :
-----------------------------------
and once this form is filled and when the user press the "SUBMIT" button, i want to recieve the filled info in my email or a database.
how can i do this?? can anyone help me out?

Any rookie php coder can do that easily.

But, to explain how would take far too long. It requires a sound knowledge of php.

The form would be simple enough, but the script above does more to cater to hosting account signups like a built in whois and payment gateway integration.

and also a built in help ticket system, one-click email invoicing, and client management.

If you don't mind spending the money, that one is much better than PHPCoin. It'll auto create accounts, a feature I never used on it, but nice to have.
