I think Paypal is the best of all and more widely used and accepted. A few alternatives to Paypal are 2Checkout, Worldpay, Ikobo etc.

Personally if I was transferring any sizeable sums, I would open up a bank account and do a bank to bank transfer. Not sure which two countries are involved but this is usually the safest and least expensive method (also keeping in mind, you can't transfer to some countries using Paypal)
If we are talking less then a hundred, etc it doesn't really matter

Living in Japan, we get people wanting to do wire transfers a lot. It turns into a pain in the ass. Usually the US bank will charge $40 to $60 for the wire transfer, and then we get charged by the Japanese bank to accept the wire transfer (another $50 or so).
PayPal doesn't charge that much, but they do tend to have a sucky exchange rate.

Bank Account is best option or you can try western union
If you are in good mood then goto market and buy some costly painting and send it thru DHL to anywhere in the world and that (wo)man can resell it to get money even some profit is possible

Banks (like everyone it seems) will always try to screw you. If you agree to transfer rates before you sign the dotted line, you can get some pretty good ones. There is so much competition banks are tripping over each other to do away with those acceptance and processing charges....Of course only to super screw the poor bastard that never asked about pricing.
Paypal is definately preffered if a. they transfer to that country and b. it isn't a whole lot of money (completely subjective of course)

Paypal is absolutely safe from the Paypal side...The only issue with using them for your site, for instance, is if you get burnt paying for non tangible goods (like a template for instance), its really hard to recover the money. For moving money though its completely safe. Just be sure you register with completely real details because they ask for additional identification like a passport when you take in more then $6500 (at least for most countries)
The only other piece of advice is never click on a link in an email which appears to be from Paypal. If there is a problem with your account, go directly to https://www.paypal.com and log in (type it in, again don't follow links) - If there is an issue they will have a notice there.

I checked into this today.... for myself
My bank wants $17 per transfer and says it takes up to 40 days because of "international wire something another". She also said that she is currently investigating a transfer where the funds never arrived... Yeah not the best option.
PayPal was not an option for me or I would use that.
Western Union charges $25/$500 for the country I was looking at sending to and you can do it online wtih your CC. This makes Western Union the best option for me, and not to much more than my bank. From the sounds of it it's cheaper than some of the other banks out there!

PayPal froze my account [this time] for having a large amount of funds transfered into it because it was not "normal" for my paypal account. They basicaly would not let me take any money out but I could still receive money.[get more $ frozen] After a week they unfroze my account... Not a very safe solution to do large sums of $ monthly you run a risk.
Also like you said for "Digital Goods" paypal wont care. I lost a couple websites and programming because paypal said they couldn't tell what I did or they didn't " know how to check " that.

Google Checkouts or Moneybookers. Don't ever deal with wire, once the trade has been done, there is no going back.

To transfer funds woldwide there are services like western union which are great but can provide you less exchange rates. And there are local services in each and every country for you to transfer the fund to that country.
But the best ones are the online ones such as paypal, and the second to it is moneybookers.
