This page doesn't have a design, what exactly do you need to have reviewed? SEO? functionality?
I mean it's just a form...

I converted a video to mp3, I think the service is a very useful one, perhaps the conversion speed is not very good but I liked it.
I think you should also add some simple design to it and maybe a search feature to make it easier to locate the videos instead of pasting the URL in the field.
Regards

The download speed was fine (110Kb/s which for my internet connection is normal), what I meant by bad speed was the conversion speed (downloading from the youtube server to your site server and converting to mp3). It took about 2 or 3 minutes to convert a video to mp3.
What library are you using for the conversion?

hmmmmm . . . may be my monitor (but I doubt it) all I can see is the silver text image of the word "Download Premium", the YouTube icon, I can faintly make out the black arrow and download icons on the DARK blue background and the reg GO icon on the black form holder.
I'm assuming you're not finished with this and plan to provide some contrasting icons and text in the form field (That I had to squint to see the words "YOU TUBE VIDEO URL HERE").
I tried this page: http://www.download-premium.com/ in FireFox, IE-7, Chrome and Opera - same story.

The functionality is good and it was very fast in both conversion and download speed. But the template design is not very impressive and looks old too.
